Appointment Booking Instructions​

To book an appointment, please follow these steps:

  1. Schedule Your Appointment: Once your booking is confirmed, a video call link will be sent to your email.

  2. Patient Portal Access: After your consultation is confirmed, your provider will send you an invitation to access the patient portal. Additionally, you will receive an intake form to collect necessary patient information and obtain consent for the consultation.

  3. Payment: Please proceed with the payment based on the services you’ve selected.

  4. Connect for Consultation: At the scheduled time, please use the provided link to join the video consultation.

Note: Some services may be covered by your insurance provider. Please verify your coverage for reimbursement.

  • Evaluation and Management (20 min-150$) (New Patient): Comprehensive assessment and management of patient health, including history, examination, and medical decision-making. 
  • Evaluation and Management (20 min-100$) (Established Patient): Comprehensive assessment and management of patient health, including history, examination, and medical decision-making. 
  • Online Digital E/M Services (10 min-20$)(Established Patient):Online evaluation and management services for established patients over a 7-day period.
  • Online Digital E/M Services (20 min-30$)(Established Patient):Online evaluation and management services for established patients over a 7-day period.
  • Online Digital E/M Services (30 min-47$)(Established Patient): Online evaluation and management services for established patients over a 7-day period.
  • Remote Patient Monitoring (RPM)(30 min-200$): Monitoring of patients’ physiological data (e.g., blood pressure, glucose) remotely.
  • Telephonic Evaluation and Management (10 min-50$)(Established Patient): Telephone consultations for established patients, providing medical advice and management. 
  • Telephonic Evaluation and Management (20 min-100$)(Established Patient): Telephone consultations for established patients, providing medical advice and management. 
  • Telephonic Evaluation and Management (30 min-150$)(Established Patient): Telephone consultations for established patients, providing medical advice and management. 
  • Virtual Check-Ins HCPCS (10 min-14$): Brief communication technology-based services for established patients, including follow-ups and check-ins.
